Before Your First Visit
New Patient Registration Form
To save time at your first appointment, please download, print, and complete your registration form in advance. Return the signed form to us before your visit using any of the options below.

- Photo ID (driver's license or state ID)
- Insurance card(s)
- List of current medications and dosages
- List of known allergies
- Any prior dermatology records or pathology reports if available
- Completed new patient forms (download below or arrive 15 minutes early to complete in office)
Insurance & Payment
Peterson Dermatology accepts most major insurance plans including Medicare and Medicaid. Please call our office at 912-538-9080 to verify that we accept your specific plan before your appointment. We also offer self-pay options — please ask our staff for details.
Office Policies
Appointments
Please arrive 15 minutes early for your first visit to complete paperwork. If you need to cancel or reschedule, please call us at least 24 hours in advance.
After-Hours Emergencies
For urgent skin concerns after hours, please call our office and follow the prompts for after-hours guidance, or visit your nearest urgent care or emergency room.
Prescription Refills
Prescription refill requests require an office visit or a recent visit within the past 12 months. Please allow 48–72 hours for refill processing.
Referrals
If your insurance requires a referral, please obtain one from your primary care physician prior to your appointment.
